99202-99215 Evaluation & Management code selection levels are now based on Total Time or level of Medical Decision Making (MDM).
Total Time is the time spent on the patient examined. This is time spent by the provider on the day of visit face-to-face and non-face-to-face (before and after visit related to your patient’s exam).
Note: Each code specifies how much time on the date of the encounter.
Medical Decision Making includes 4 levels:
1. 99202/99212: Straightforward (1 self-limited or minor problem)
2. 99203/99213: Low (2 or more self-limited or minor problems)
3. 99204/99214: Moderate (1 acute complicated injury)
4. 99205/99215: High (serious high risk and very rare for chiropractor)
